Personal Flotation Device (PFD) Type V [Rental]

This PFD is available in three standardized size ranges to ensure proper fit, flotation performance, and safety compliance:

• XS / S

• M / L

• XL / XXL

Sizing is issued based on availability and participant selection. Proper fit is the responsibility of the participant at the time of issue.

Participants must confirm that the issued PFD fits properly before entering any training or operational activity.

You are responsible for:

• Checking fit, strap adjustment, and closure security

• Confirming unrestricted breathing and movement

• Notifying instructors immediately if the fit is improper

Once training begins, the equipment is considered accepted as fit for use.

Total amount required at checkout: 4,000 THB

• 2,500 THB – Equipment rental fee (per course)

• 1,500 THB – Refundable security deposit

Rental fee and deposit are separate charges collected together.

This amount consists of two separate components:

1. Equipment Provision Fee

2,500 THB – per course

This fee covers:

• Use of MedPro Extreme professional-grade PFD

• Cleaning, inspection, and maintenance before and after use

• Equipment readiness and compliance checks

This fee is non-refundable once the equipment has been issued.

2. Refundable Security Deposit

1,500 THB – per unit

The deposit is collected separately and is intended to cover:

• Damage beyond normal training wear

• Loss of equipment or components

• Equipment returned in unsafe or non-operational condition

The deposit is refundable after equipment return and inspection, minus any applicable charges.

• The rental period begins on the date the equipment is dispatched from the MedPro Extreme warehouse

• Equipment must be returned within 7 calendar days from the dispatch date

• Equipment must be returned to the designated MedPro Extreme warehouse or return location

Late Return Policy

• Late returns are charged at 300 THB per day, per item

• Late fees may be deducted from the security deposit

• Continued non-return may result in full replacement charges

Participants are fully responsible for the equipment from the moment of issue until it is returned and cleared by inspection.

Damage, loss, or unsafe condition will be charged according to the MedPro Extreme Damage & Replacement Cost Table.

Charges may include, but are not limited to:

• Surface damage

• Strap or buckle damage

• Buoyancy degradation

• Structural or shell failure

• Loss of the entire unit

If damage or loss exceeds the security deposit amount, the remaining balance must be paid by the participant.

Equipment deemed unsafe for further use will be charged at full replacement value, regardless of partial damage.

This PFD is designed and approved for:

• Whitewater rescue operations

• Swiftwater and flood response

• Sea-based rescue training

• SUP and rafting environments

• Professional rescue and medical training scenarios

Use outside of intended training activities or misuse that leads to damage remains the responsibility of the participant.

Participants agree to:

• Use the equipment as instructed

• Avoid unnecessary abuse or misuse

• Report any damage immediately

• Return equipment clean and intact

Failure to report damage may result in additional charges.

By proceeding with equipment provision, participants acknowledge and agree to comply with:

• MedPro Extreme Privacy Policy

All policies are available for review on the MedPro Extreme official website and apply in full to this equipment provision.

Failure to return equipment within the allowed period may result in:

• Continued daily charges

• Forfeiture of the security deposit

• Full replacement charges if equipment is not recovered

MedPro Extreme reserves the right to pursue recovery costs where necessary.
